Do you require a Mental Health Care Plan? (MHCP) *

A Mental Health Care Plan is not required to see a psychologist. However in order to receive a Medicare rebate for psychological services, a MHCP is required.

Important Information About your MHCP

When you see your GP for a Mental Health Care Plan to ensure it is a valid referral please request they include:

Date of Referral, Patient’s name, Patient’s date of birth , Patient’s address, Diagnosis, Request for services under Better Access, Number of sessions requested, Referring practitioner’s Medicare Provider Number, Practitioner’s signature
